GROUP TRAINING WAIVER

You have chosen to participate in exercise training with Tanya Jones. Exercise activity should not be undertaken without first consulting a physician, especially if you are pregnant or have any physical limitations due to age or disease or are taking medications. Please discontinue any activity that causes you pain or severe discomfort and consult a physician.

  1. Has your doctor ever said that you have a heart condition and that you should only do physical activity recommended by a doctor?

  2. Do you feel pain in your chest when you do physical activity?

  3. In the past month, have you had chest pain when you were not doing physical activity?

  4. Do you lose your balance because of dizziness or do you ever lose consciousness?

  5. Do you have a bone or joint problem that could be made worse by a change in your physical activity?

  6. Is your doctor currently prescribing drugs (for example, water pills) for your blood pressure or heart condition?

  7. Do you know of any other reason why you should not do physical activity?

If you answered YES to any of the above questions, please fill out the following PAR-Q plus and email to tanya@bee3ftness.com.

THIS DOCUMENT IS A RELEASE OF CLAIMS AND BY SIGNING IT YOU ACKNOWLEDGE AND AGREE TO THE FOLLOWING:

  1. I acknowledge that all exercise activity, including the use of equipment, is a potentially hazardous activity. I also understand that fitness activities involve a risk of injury and even death, and that I am voluntarily participating in these activities and using equipment and machinery with knowledge of the dangers involved.

  2. I am in good health and sufficient physical condition to engage in exercise activity and am voluntarily participating in this activity.

  3. In consideration for being allowed to participate, I agree to assume all risks of exercise. I agree to release and hold harmless Tanya Jones and any representative of “Bee 3 Fitness” from any liability or responsibility for any physical injury, illness or death you may incur during, or as a result of participating in this exercise activity.

  4. I agree for myself and on behalf of my guests, heirs, representatives, successors and assigns that Tanya Jones and those working at “Bee 3 Fitness” will not be liable for any damages or injuries I or User Parties may suffer in or about “Bee 3 Fitness” facilities.

  5. I hereby grant permission to “Bee 3 Fitness” of Surrey, BC. to use my photograph, video and testimonial that I might provide, on its web site or in other official publications.

    This is a standing waiver that applies to all future classes you take with Tanya Jones
